1. (a) List of the directors of the applicant, showing the name of each director.
2. (b) List of the officers of the applicant, showing the name and title of each officer.
3. (c) Balance sheet of the applicant as of a date within ten days before the filing of the report.

Not more than 30 days nor less than five business days before an applicant is issued a license, such applicant shall file with the Commissioner a report containing the following information:
