1. (a) Appointment of Commissioner of Business Oversight as Attorney for Service of Process in the form of Department Form 85, completed and signed in accordance with the instructions for such Form.
2. (b) Copy of a resolution of the board of the applicant, authorizing the signing of the Appointment called for in Subdivision (a) of this Section. Such copy of the resolution of the board of the applicant shall be certified by an officer of the applicant who is assigned to the head office of the applicant and who is not proposed to be assigned to the subject office, and the signature of such officer on such certification shall be acknowledged.

Except as otherwise provided in Section 10.14176 of this Chapter, not less than five business days before the license for a subject office is to be issued, the applicant shall file with the Commissioner:
