1. (a) Appointment of Commissioner of Business Oversight as Attorney for Service of Process, in the form of Department Form 75, completed and signed in accordance with the instructions for such Form.
2. (b) Copy of a resolution of the board of the applicant, authorizing the signing of the Appointment called for in Subdivision (a) of this Section. Such copy of the resolution of the board of the applicant shall be certified by an officer of the applicant who is assigned to the head office of the applicant and who is not proposed to be assigned to the representative office, and the signature of such officer on such certification shall be acknowledged.

Not less than five business days before the license for a California state representative office is to be issued, the applicant shall, unless it has already done so, file with the Commissioner:
