1. (a) Surrender to the Commissioner for cancellation the certificate of authority authorizing the subject institution to establish and maintain the place of business.

2. (b) File with the Commissioner a report containing the following information:

   1. (1) Identify the place of business.
   2. (2) State the date on which the place of business was discontinued.

Not more than five business days after discontinuing a place of business, a subject institution shall:
