1. (a) Not less than one business day before establishing a branch office, an applicant shall file with the Commissioner an application for the issuance of a certificate of authority authorizing such applicant to establish and maintain such branch office. Such application shall contain the following information:

   1. (1) Identify the branch office.
   2. (2) State the date on which the applicant proposes to establish the branch office.
2. (b) An applicant may establish a branch office on or after the date on which the certificate of authority authorizing such applicant to establish and maintain such branch office is issued.
