1. (a) Not less than one business day before commencing business, a subject institution shall file with the Commissioner an application for the issuance of a certificate of authority authorizing such subject institution to transact commercial banking business, trust business, or both, as the case may be. Such application shall state:

   1. (1) Address of the head office of the subject institution.
   2. (2) Date on which the subject institution proposes to commence business.
2. (b) The application called for in Subdivision (a) of this Section shall be accompanied by the fee prescribed in Financial Code Section 400(c).
