Ariz. Rev. Stat. § 45-703 – Application for certificate of groundwater oversupply; fee

A. The director shall prescribe and furnish an application form for a certificate of groundwater oversupply that includes the following:
1. 1. The name and mailing address of the district member.
2. 2. The name of the active management area in which the groundwater will be withdrawn.
3. 3. The estimated annual quantity of groundwater to be designated as groundwater oversupply.
4. 4. The locations and well registration numbers of the wells from which the designated groundwater will be withdrawn.
5. 5. The proposed duration of the certificate.
6. 6. Evidence of the district member's right under articles 4 through 7 of this chapter to withdraw the groundwater to be designated as groundwater oversupply.
7. 7. Evidence that the groundwater to be designated will be withdrawn from an area in the district of localized excess groundwater.
8. 8. Evidence that if the district member does not withdraw the groundwater to be designated either of the following could occur in the active management area in which the district is located:
  1. (a) The groundwater would be discharged from the district without being used and would not contribute hydrologically to achieving safe-yield.
  2. (b) The groundwater would contribute to a waterlogging or drainage problem.
9. 9. Any other information the director may reasonably require.
B. The director shall establish and collect a reasonable fee from the applicant to cover the cost of administrative services and other expenses associated with evaluating and issuing a certificate of groundwater oversupply. The director shall deposit, pursuant to sections 35-146 and 35-147, all fees received under this subsection in the water resources fund established by section 45-117.
