A. The diabetes action plan team is established in the department of health services. The team is composed of the head of the following entities or that person's designee:

1. 1. The Arizona diabetes program within the department of health services.
2. 2. The Arizona health care cost containment system.
3. 3. The public safety personnel retirement system.
4. 4. The Arizona state retirement system.
5. 5. The department of administration benefits services division.
6. 6. Diabetes stakeholder organizations, including health insurers, a nationally recognized diabetes association and the Arizona diabetes coalition.

B. The team shall compile a report once every two years that includes the following information:

1. 1. The prevalence in this state of:

   1. (a) Diabetes by type.
   2. (b) Diabetes by age, race and gender.
   3. (c) Complications associated with diabetes.
   4. (d) Prediabetes.
2. 2. The costs of diabetes in this state.
3. 3. The Arizona diabetes program's plan for reducing the incidence of diabetes in this state, improving diabetes care and reducing diabetes-related health disparities, including proposed action steps.
4. 4. A description of the level of coordination that exists between the department of health services and hospitals, the department's contracted partners and other stakeholders on activities and programmatic activities and the level of communication on managing, treating or preventing all forms of diabetes and its complications.
