The board shall establish and collect fees of not more than:
- 1. $300 for an application for an original license. This fee is nonrefundable.
- 2. $300 for a certificate of renewal of a license.
- 3. $300 for an application for reinstatement of licensure.
- 4. $50 for each duplicate license.
