B. Wages do not include:
1. For the purpose of sections 23-604, 23-726 and 23-728, that part of the remuneration, other than remuneration referred to in paragraphs 3 through 16 of this subsection, in excess of:

The amount of any payment, including monies paid by an employer for insurance or annuities or into a fund to provide payments for insurance or annuities, made to or on behalf of an employee or any of the employee's dependents under a plan or system established by an employer that makes provision for the employer's employees generally, for the employer's employees generally and their dependents, for a class of the employer's employees or for a class of the employer's employees and their dependents, on account of any of the following:
