1. A. Upon receipt of a complaint or information indicating that a group home or behavioral-supported group home may not be in compliance with A.R.S. Title 36, Chapter 5.1 or this Chapter, the Department may:

   1. 1. Investigate the complaint or information about noncompliance within 30 days after receipt of the complaint or information about noncompliance;
   2. 2. Develop a written report documenting the investigation;
   3. 3. Provide the licensee with the written report in subsection (A)(2); and
   4. 4. If the complaint or information about noncompliance was substantiated, notify the Division of the outcome of the investigation.

2. B. If the Department substantiates a complaint or information about noncompliance at a group home or behavioral-supported group home, the licensee of the group home or behavioral-supported group home shall:

   1. 1. Establish a plan of correction, if applicable, for correction of a deficiency;
   2. 2. Agree to carry out the plan of correction by signing the written report in subsection (A)(2); and
   3. 3. Ensure that a deficiency listed on the plan of correction is corrected within 30 days after the date of the plan of correction or within a time period the Department and the licensee agree upon in writing.
