1. A. An applicant shall obtain a license to operate a group home or behavioral-supported group home from the Department before providing supervision or habilitation to an individual with a cognitive disability, developmental disability, dual diagnosis, or challenging behavior in a group home or behavioral-supported group home.

2. B. A license to operate a group home or behavioral-supported group home is valid for the following, as indicated on the license:

   1. 1. Address of the group home or behavioral-supported group home;
   2. 2. Name of the licensee;
   3. 3. Name of the group home or behavioral-supported group home, if applicable;
   4. 4. Fire risk prevention level; and
   5. 5. Licensing period for the group home or behavioral-supported group home.
