1. 1. Reside in Arizona;
2. 2. Be a citizen of the U.S. or an alien whose presence in the U.S. is authorized under federal law;
3. 3. Have a need for telecommunications equipment available through the Program due to a hearing or speech-related disability, as certified by an authorized person described in R9-26-203;
4. 4. Have access to a telephone line; and
5. 5. Not have used a voucher to purchase telecommunications equipment that is still under warranty unless the individual’s disability status changed during the warranty period.

To be eligible for telecommunications equipment through the Program, a person shall:
