1. A. No later than 30 days after the date an EMCT’s name legally changes, the EMCT shall submit to the Department:

   1. 1. The following information in a Department-provided format:

      1. a. The name under which the EMCT is currently certified by the Department;
      2. b. The EMCT’s address, telephone number, and Social Security number; and
      3. c. The EMCT’s new name; and
   2. 2. Documentation showing that the name has been legally changed.

2. B. No later than 30 days after the date an EMCT’s address or email address changes, the EMCT shall submit to the Department the following information in a Department-provided format:

   1. 1. The EMCT’s name, telephone number, and Social Security number; and
   2. 2. The EMCT’s new address or email address.

3. C. An EMCT shall notify the Department in writing no later than 10 days after the date the EMCT:

   1. 1. Is incarcerated or is placed on parole, supervised release, or probation for any criminal conviction;

   2. 2. Is convicted of:

      1. a. A crime specified in R9-25-402(A)(2),
      2. b. A misdemeanor involving moral turpitude,
      3. c. A felony in this state or any other state or jurisdiction, or
      4. d. A misdemeanor specified in R9-25-402(E);
   3. 3. Has registration revoked or suspended by a national certification organization;
   4. 4. Has certification, recertification, or licensure at an EMCT classification level revoked, suspended, or voluntarily surrendered in another state or jurisdiction; or
   5. 5. Has certification or licensure as a health professional, as defined in A.R.S. § 36-3201, revoked, suspended, or voluntarily surrendered in Arizona or in another state or jurisdiction.
