1. 1. The amount of funding available for the Arizona Dental Sealant Program,
2. 2. The time and date the Department received the application,
3. 3. The school’s percentage of children attending the school that participated in the National School Lunch Program, and
4. 4. Whether the school has at least 25 children in second and sixth grades when the number of children in each grade is added together.

The Department uses the following criteria when determining whether to approve a school for participation in the Arizona Dental Sealant Program:
