1. A. A certificate of fetal death registration contains, as available, the information specified in R9-19-305(B)(1).
2. B. A certificate of birth resulting in stillbirth contains, as available, the information specified in R9-19-305(B)(1)(a) through (k) and (n).

3. C. A parent of a fetus or a person who is of legal age and who is authorized by a parent of the fetus may request a certified or noncertified copy of a certificate of fetal death registration for the fetus by submitting to the State Registrar or a local registrar:

   1. 1. A written request, in a Department-provided format, that includes:

      1. a. The name and mailing address of the person submitting the request;
      2. b. Contact information for the person submitting the request, which includes a telephone number or an e-mail address;
      3. c. Whether the person submitting the request is a parent of a fetus or a person authorized by a parent of the fetus;

      4. d. The following information:

         1. i. The name of the mother in the registered fetal death record, and
         2. ii. The date of delivery;

      5. e. If known, the:

         1. i. State file number,
         2. ii. Town or city of the fetal death, and
         3. iii. County of the fetal death;
      6. f. If the person submitting the request is a parent of the fetus, whether the person would like to receive a certified copy of a certificate of birth resulting in stillbirth for the fetus;

      7. g. The number being requested of:

         1. i. Certified copies of a certificate of fetal death registration,
         2. ii. Noncertified copies of a certificate of fetal death registration, and
         3. iii. Certified copies of a certificate of birth resulting in stillbirth; and
      8. h. The dated signature of the person submitting the request, either:

      9. i. With the person’s signature notarized; or

         1. ii. Accompanied by a copy of a valid, government-issued form of photo identification for the person that contains the name and signature of the person;
   2. 2. For a parent whose name is not included in the registered fetal death record, documentation demonstrating that the person submitting the request is a parent of the fetus;

   3. 3. For a person authorized by a parent of the fetus to receive a certified or noncertified copy of the certificate of fetal death registration for the fetus:

      1. a. Documentation demonstrating that the person submitting the request is authorized to receive a certified or noncertified copy of a certificate of fetal death registration for the fetus; and
      2. b. Documentation demonstrating that the individual authorizing the person submitting the request to receive a certified or noncertified copy of a certificate of fetal death registration for the fetus is a parent of the fetus; and
   4. 4. The applicable fee in R9-19-105 for each certificate of being requested.
