1. 1. Maintain a copy of the following documents at the place of final disposition for at least five years after the issue date on the document:

   1. a. The disposition-transit permit for each final disposition of human remains, and
   2. b. The disinterment-reinterment permit for each disinterment or reinterment of human remains; and
2. 2. Provide a copy of the document to the State Registrar for review within 48 hours after the State Registrar’s request.
