Ariz. Admin. Code § R9-16-315
A. A hearing aid dispenser licensee or temporary hearing aid dispenser licensee shall submit a change application to the Department in a Department-provided format, within 30 calendar days after the effective date of a change in:

B. A licensee may obtain a duplicate license and update the information described in subsection (A) by submitting to the Department a request for a change application in a Department-provided format that includes:
