1. A. In addition to complying with R9-16-303, an applicant for initial licensure by examination shall submit an application to the Department that includes:

   1. 1. Information and documentation required in R9-16-303, and
   2. 2. The name and hearing aid dispenser or dispensing audiologist license number of the individual responsible for providing the attestation described in R9-16-302(B)(4).
2. B. If the Department approves the application, the Department shall notify the applicant of approval to take the written hearing aid dispenser examination as specified in R9-16-302.
3. C. If the Department approves an application, the applicant shall not practice fitting and dispensing hearing aids without a license issued by the Department.
