1. 1. Establish a record for each speech-language pathologist assistant who receives direct supervision and indirect supervision from the speech-language pathologist that includes:

   1. a. The speech-language pathologist assistant’s license number, name, home address, telephone number, and e-mail;
   2. b. A plan indicating the types of skills and the number of hours allocated to the development of each skill that the speech-language pathologist assistant is expected to complete;

   3. c. A document listing each occurrence of direct supervision or indirect supervision provided to the speech-language pathologist assistant that includes:

      1. i. Business name and address where supervision occurred,
      2. ii. The date and times when the supervision started and ended,
      3. iii. The types of clinical interactions provided, and
      4. iv. Notation of speech-language pathologist assistant’s progress;
   4. d. Documentation of evaluations provided to the speech-language pathologist assistant during the time supervision was provided; and
   5. e. Documentation of when supervision was terminated; and

2. 2. Maintain a speech-language pathologist assistant record:

   1. a. Throughout the period that the speech-language pathologist assistant receives direct supervision and indirect supervision clinical interactions from the supervisor; and
   2. b. For at least two years after the last date the speech-language pathologist assistant received clinical interactions from the supervisor.

A licensed speech-language pathologist who provides direct supervision or indirect supervision to a speech-language pathologist assistant shall comply with A.R.S. § 36-1940.04(F) and (G):
