1. 1. Radiology services and diagnostic imaging services are provided in compliance with A.R.S. Title 30, Chapter 4 and 9 A.A.C. 7;
2. 2. A copy of a certificate documenting compliance with subsection (1) is maintained by the nursing care institution;
3. 3. When needed by a resident, radiology services and diagnostic imaging services delineated in the nursing care institution’s scope of services are provided on the nursing care institution’s premises;

4. 4. Radiology services and diagnostic imaging services are provided:

   1. a. Under the direction of a physician; and

   2. b. According to an order that includes:

      1. i. The resident’s name,
      2. ii. The name of the ordering individual,
      3. iii. The radiological or diagnostic imaging procedure ordered, and
      4. iv. The reason for the procedure;
5. 5. A medical director, attending physician, or radiologist interprets the radiologic or diagnostic image;

6. 6. A radiologic or diagnostic imaging report is prepared that includes:

   1. a. The resident’s name;
   2. b. The date of the procedure;
   3. c. A medical director, attending physician, or radiologist’s interpretation of the image;
   4. d. The type and amount of radiopharmaceutical used, if applicable; and
   5. e. The resident’s adverse reaction to the radiopharmaceutical, if any; and
7. 7. A radiologic or diagnostic imaging report is included in the resident’s medical record.

If radiology services or diagnostic imaging services are authorized to be provided on a nursing care institution’s premises, an administrator shall ensure that:
