1. 1. In a Department-provided format whether the applicant:

   1. a. Has:

      1. i. A secured area for a resident with Alzheimer’s disease or other dementia, or
      2. ii. An area for a resident on a ventilator;

   2. b. Is requesting authorization to provide to a resident:

      1. i. Behavioral health services,
      2. ii. Clinical laboratory services,
      3. iii. Dialysis services, or
      4. iv. Radiology services and diagnostic imaging services; and
   3. c. Is requesting authorization to operate a nutrition and feeding assistant training program; and
2. 2. If the governing authority is requesting authorization to operate a nutrition and feeding assistant training program, the information and documentation in R9-10-116(B)(1)(a), (B)(1)(c), and (B)(2).

In addition to the license application requirements in A.R.S. § 36-422 and R9-10-105, an applicant for a license as a nursing care institution shall include:
