1. A. In addition to the license application requirements in A.R.S. § 36-422 and R9-10-105, an applicant for a license as a nursing-supported group home shall include:

   1. 1. In a Department-provided format, whether the applicant is requesting authorization:

      1. a. To admit residents who:

         1. i. Are on a ventilator,
         2. ii. Have a tracheostomy tube, or
         3. iii. Receive total parenteral nutrition; or

      2. b. To provide:

         1. i. Services to individuals under 18 years of age, including the licensed capacity requested;
         2. ii. Restraint;
         3. iii. Clinical laboratory services; or
         4. iv. Respiratory care services; and
   2. 2. A copy of the applicant’s service provider award letter with the Division.

2. B. A licensee shall submit to the Department, with the relevant fees required in R9-10-106(C) and in a Department-provided format:

   1. 1. The information required in subsection (A)(1), as applicable; and
   2. 2. Documentation of the licensee’s service provider contract with the Division.
