1. A. A licensee shall ensure that the premises:

   1. 1. Provide lighting and ventilation to ensure the health and safety of a patient;
   2. 2. Are maintained in a clean condition;
   3. 3. Are free from a condition or situation that may cause a patient to suffer physical injury;
   4. 4. Are maintained free from insects and vermin;
   5. 5. Are smoke-free; and

   6. 6. Are sufficient to accommodate:

      1. a. The services stated in the pain management center’s scope of services, and
      2. b. An individual accepted as a patient by the pain management center.

2. B. A licensee shall ensure that if a pain management clinic collects urine specimens from a patient, the pain management clinic has at least one bathroom on the premises that:

   1. 1. Contains:

      1. a. A working sink with running water,
      2. b. A working toilet that flushes and has a seat,
      3. c. Toilet tissue,
      4. d. Soap for hand washing,
      5. e. Paper towels or a mechanical air hand dryer,
      6. f. Lighting, and
      7. g. A means of ventilation; and
   2. 2. Is for the exclusive use of the pain management clinic.
