1. 1. If rehabilitation services are provided as an organized service, the rehabilitation services are provided under the direction of an individual qualified according to policies and procedures;
2. 2. Rehabilitation services are provided according to an order; and

3. 3. The medical record of a patient receiving rehabilitation services includes:

   1. a. An order for rehabilitation services that includes the name of the ordering individual and a referring diagnosis,
   2. b. A documented care plan that is developed in coordination with the ordering individual and the individual providing the rehabilitation services,
   3. c. The rehabilitation services provided,
   4. d. The patient’s response to the rehabilitation services, and
   5. e. The authentication of the individual providing the rehabilitation services.
