1. 1. Respiratory care services are provided under the direction of a medical staff member;

2. 2. Respiratory care services are provided according to an order that includes:

   1. a. The patient’s name;
   2. b. The name and signature of the ordering individual;
   3. c. The type, frequency, and, if applicable, duration of treatment;
   4. d. The type and dosage of medication and diluent; and
   5. e. The oxygen concentration or oxygen liter flow and method of administration;

3. 3. Respiratory care services provided to a patient are documented in the patient’s medical record and include:

   1. a. The date and time of administration;
   2. b. The type of respiratory care services;
   3. c. The effect of respiratory care services;
   4. d. If applicable, any adverse reaction to respiratory care services; and
   5. e. The authentication of the individual providing the respiratory care services; and
4. 4. Any area or unit that performs blood gases or clinical laboratory tests complies with the requirements in R9-10-219.

An administrator of a hospital that provides respiratory care services shall ensure that:
