1. A. A provider shall ensure that behavioral health services and ancillary services are provided to a recipient according to the recipient’s treatment plan obtained from the behavioral health respite home’s collaborating health care institution.

2. B. A provider shall submit to the behavioral health respite home’s collaborating health care institution and, if applicable, the recipient’s case manager:

   1. 1. Documentation of any significant change in a recipient’s behavior or physical, cognitive, or functional condition and the action taken by a provider to address the recipient’s changing needs; and
   2. 2. Notification of a recipient’s unexpected self-release.
