- 2. By hand delivery to the Department’s offices at 4814 South 40th Street, Phoenix, Arizona 85040; or
- 3. Where practicable, by electronic submission, for example, Electronic Information Exchange, or CD–ROM. Electronic submissions shall be made in a manner that enables the Department to receive, read, authenticate, distribute, and archive the submission, and process and retrieve it a single page at a time.
