1. 1. Name, age, address, and telephone number of the individual exposed;
2. 2. Date of report;
3. 3. Reporting institution or physician;
4. 4. Date of exposure;
5. 5. Body part exposed;
6. 6. Type of exposure: Bite or saliva contact (non-bite);
7. 7. Species of animal;
8. 8. Animal disposition: quarantined, euthanized, died, unable to locate;
9. 9. Animal rabies test results, if any: positive or negative;
10. 10. Treatment regimen; and
11. 11. Date treatment was initiated.

A physician or an authorized designee shall submit a written or electronic report to the Department for each individual exposed who receive post-exposure rabies prophylaxis that includes:
