1. A. Case control measures: A local health agency shall:

   1. 1. Conduct an epidemiologic investigation of each reported blastomycosis case or suspect case; and
   2. 2. For each blastomycosis case, submit to the Department, as specified in Table 2.4, the information required under R9-206(D).

2. B. Outbreak control measures: A local health agency shall:

   1. 1. Conduct an epidemiologic investigation of each reported outbreak of blastomycosis; and
   2. 2. For each outbreak of blastomycosis, submit to the Department the information required under R9-6-206(E).
