1. 1. An application, in a Department-provided format that contains:

   1. a. The licensee’s name,
   2. b. The facility’s name and license number, and

   3. c. Whether the licensee intends to submit the applicable fee:

      1. i. With the form, or
      2. ii. According to the payment plan in subsection (2)(b), and

2. 2. Either:

   1. a. The applicable fee, as specified in R9-5-206, or
   2. b. One-half of the applicable fee in R9-5-206 with the application and the remainder of the applicable fee due no later than 120 calendar days after the anniversary date of the facility’s license.

A licensee shall submit the following to the Department, on an annual basis, no more than 60 calendar days before the anniversary date of the facility’s license:
