1. 1. The certificate holder or a staff member immediately reports suspected abuse or neglect of an enrolled child under A.R.S. Title 8, Chapter 4, Article 8, or to a local law enforcement agency, as required by A.R.S. § 13-3620;
2. 2. If a staff member or resident is suspected of abuse or neglect of an enrolled child, the certificate holder also reports the suspected abuse or neglect to the Department; and
3. 3. Documentation of a report required in subsection (1) or (2) is maintained on the premises for 12 months after the date of the report.
