1. 1. All information as prescribed in A.R.S. § 46-454(C); and,

2. 2. As much information regarding the allegedly incapacitated, or vulnerable adult as is available to the source including:

   1. a. The names and addresses of those involved and their roles;
   2. b. The length of time the situation has been ongoing;
   3. c. The client’s functional level;
   4. d. Whether other agencies are providing assistance and, if so, what type of assistance; and,
   5. e. Any other information that may assist the APS worker in the investigation.

Upon receipt of a report, as prescribed in A.R.S. § 46-454, APS shall ask the reporting source to provide:
