1. 1. The pursuit of evaluations and professional assessments necessary to substantiate the need for services;
2. 2. The collection and analysis of information regarding eligibility and the prioritization of service needs;
3. 3. The provision of information on available services and referral to appropriate service alternatives; and
4. 4. The development of individual habilitation goals and objectives for the client through the ISPP.

Upon the filing of an application for admission to services, the Division shall assign a case manager to assist the applicant. Upon admission, the case manager shall assist the client and the client’s family in all aspects of the developmental disabilities service delivery system as follows:
