1. 1. Lawful presence of the person to whom services are to be provided, as required by A.R.S. § 1-502.

2. 2. Residency. An applicant shall:

   1. a. Verify current residency and intent to remain in Arizona by signing the application.
   2. b. At the request of the Department, provide additional documentation demonstrating Arizona residency of the person to whom services would be provided.
3. 3. Health Insurance Coverage. An applicant shall provide information regarding current health insurance coverage for the person to whom services would be provided, as required in Article 13 of this Chapter.
