1. A. Every regular license shall expire one year from the date of issuance and may be renewed annually upon application of the Child Placing Agency.

   1. 1. License renewal is not automatic.

   2. 2. License renewal requires:

      1. a. A consultation;
      2. b. An application;
      3. c. A written description of services provided; and
      4. d. Licensing study (see R6-5-6904(E)).
   3. 3. For license renewal, each Child Placing Agency must meet all standards for licensing as specified in this Article.

2. B. An application for the renewal for a Child Placing Agency shall be made in the same manner as the original application. A licensee shall reapply when:

   1. 1. The present license will expire within 30 days to 60 days; or
   2. 2. There is a plan to move within 30 days from the address on the current license; or
   3. 3. There is substantial material change in the program and/or purpose of the Child Placing Agency.
