3. 3. Pool installation;
4. 4. Relocating to a new residence;
5. 5. Change in household composition;
6. 6. Telephone number change;
7. 7. Change of backup provider;
8. 8. Voluntarily relinquishing the certificate; and
9. 9. Any other change in the home facility or the provider’s personal circumstances that affect the provider’s ability to provide stable child care services.

At least 15 days before the effective date of any scheduled change, or within 24 hours after an unscheduled change, which significantly affects the provision of child care services, a provider shall furnish the Department with written notice of the change.
