Ariz. Admin. Code § R6-5-3304
1. To open an Account in the Program, an individual shall submit a completed
application form, pay the application fee, if any, and pay an initial minimum contribution to the Account, if any, to the Program Manager at �https://az-able.com/.
3. The content of the application form shall be prescribed by the Program Manager, but shall include at a minimum, the following information:

established by the Department and the Program Manager shall be rejected. Reapplication is permissible.
