1. A. To be eligible to obtain a license or certificate by reciprocity in the same classification, as specified in R4-46-201(A), in which an individual is currently licensed or certified, the individual shall submit:

   1. 1. Evidence that the applicant is licensed or certified in a state that meets the standards established at A.R.S. § 32-3618;
   2. 2. A completed application form;
   3. 3. Disclosure of the state or states in which the individual is currently licensed or certified;
   4. 4. Evidence that the individual has applied for a valid fingerprint clearance card pursuant to A.R.S. § 32-3620(B); and
   5. 5. The application and biennial National Registry fees specified under R4-46-106.

2. B. The Department shall verify the following information:

   1. 1. License or certification number;
   2. 2. Classification, as specified in R4-46-201(A), in which the individual is currently licensed or certified; and
   3. 3. Whether the license or certificate is in good standing.
