1. A. Manufacturers, installers, and retailers (except those with a D-8B license classification), shall submit the applicable surety bond amount from the list in R4-34-403, with a form provided by the Office of Administration.

2. B. A rider to the bond is required for the following changes:

   1. 1. Location of the licensee’s principal place of business,
   2. 2. Business name,
   3. 3. Branch address,
   4. 4. License classification, or
   5. 5. Bond amount.
