1. 1. Submit to the Board a photocopy of the front and back of a new fingerprint clearance card issued to the administrator or manager during the biennial period, or

2. 2. Provide written notice to the Board if:

   1. a. The fingerprint clearance card of the administrator or manager is suspended or revoked, or
   2. b. The administrator or manager is denied a new fingerprint clearance card.

Under A.R.S. § 36-446.04, an administrator or manager is required to maintain a valid fingerprint clearance card during the biennial period. Within 10 days after the referenced action, an administrator or manager shall:
