1. A. A party may represent themselves, appoint someone to represent them, or be represented by a member of the State Bar of Arizona.
2. B. Any person intending to represent a party in an investigation or audit shall submit to the Department a completed Notice of Representation.

3. C. Any person representing a party at a contested case hearing or an appealable agency action as counsel shall properly file a Notice of Appearance which shall advise the Department of the person’s intent to appear on behalf of a party. The notice shall be filed with the Office of Administrative Hearings and served on all parties and shall contain:

   1. 1. The title of the case,
   2. 2. The name of the agency ordering the hearing,
   3. 3. The current address and telephone number of the person appearing, and
   4. 4. The name of the party for whom the person is appearing.
