1. A. A licensee shall submit a written report of completed continuing education with a renewal application that includes:

   1. 1. The name of the licensee,
   2. 2. The title of each continuing education,
   3. 3. A description of the continuing education’s content and educational objectives,
   4. 4. The date of completion of each continuing education,
   5. 5. The number of credit hours of each continuing education, and
   6. 6. A statement signed by the licensee verifying the information in the report.

2. B. The Board may audit continuing education reports every 12 months for conformance with A.R.S. § 32-829 and this Article:

   1. 1. Randomly; or
   2. 2. Selectively for licensees who previously submitted reports that did not conform with the requirements in A.R.S. § 32-829 or this Article.
