- A. As required by A.R.S. § 39-121.01, a record of Board disciplinary actions, including a decree of censure, is a public record open to public inspection.
- B. If the Board decides to restrict a license, the Board shall ensure that the restriction and any required corrective action address the conduct that led to the restriction and protect the public. If the Board decides to require that an individual with a restricted license be supervised during the period of restriction, the Board shall appoint an unrestricted licensee to provide the supervision.
- C. A physical therapist or physical therapist assistant whose license is suspended, revoked, or voluntarily surrendered shall return the license to the Board within 10 days after receipt of the Board’s final order.
- D. At the end of a period of license restriction, the Board shall terminate the restriction only if the licensee submits to the Board evidence of having completed all required corrective actions and complied with all terms of the restriction. If the Board believes it will help the Board determine whether to terminate a restriction, the licensee shall appear before the Board.
- E. An applicant who had a previous license revoked by the Board shall appear before the Board before the Board acts on the application.
