1. 1. An applicant’s or licensee’s:

   1. a. Social Security number;
   2. b. Home address or home telephone number unless the applicant or licensee designates the information for disclosure under A.R.S. § 32-3226;
   3. c. Credential evaluation report, education transcript, grades, or examination scores;
   4. d. National physical therapist or physical therapist assistant examination score;
   5. e. Diagnosis and treatment records; and
2. 2. According to A.R.S. § 32-2045, information or a document related to investigations by the Board until the information or document becomes a public record or as required by law.
