1. A. Except as provided in A.R.S. § 32-1928(B), the Board shall revoke, suspend, place on probation, or fine a licensee or permittee only after:

   1. 1. Notice is served under this Section, and
   2. 2. A hearing is conducted under R4-23-122.

2. B. The Board shall give notice of hearing to a party at least 30 days before the date set for the hearing in the manner described in R4-23-115(E) and (F). The notice shall include:

   1. 1. A statement of the date, time, place, and nature of the hearing;
   2. 2. A statement of the legal authority and jurisdiction for the hearing;
   3. 3. A reference to the particular section or sections of statute and rule involved; and
   4. 4. A statement of the violation or issue asserted by the Board.
