1. 1. A renewal form, provided by the Board, that is signed and dated by the applicant and contains the applicant’s:

   1. a. Name,
   2. b. Social Security number,
   3. c. Residence and naturopathic practice addresses, and
   4. d. Telephone number; and
2. 2. The fee required by the Board under A.R.S. § 32-1527.
