1. A. To meet the requirements in A.R.S. §§ 32-1322(A), 32-1324, or 32-1325.01(B)(2), an intern, apprentice embalmer, or embalmer’s assistant shall work a minimum of 40 hours each week and a minimum of 160 hours each month during an internship or apprenticeship.

2. B. As required in A.R.S. § 32-1330, an intern, an apprentice embalmer, or an embalmer’s assistant shall submit the following on a form provided by the Board:

   1. 1. The name of the intern, apprentice embalmer, or embalmer’s assistant;
   2. 2. The name of the funeral establishment employing the intern, apprentice embalmer, or embalmer’s assistant;
   3. 3. The supervising embalmer’s name and license number;
   4. 4. The beginning and ending dates being covered by the report;
   5. 5. The number of hours worked each week at the employing funeral establishment;

   6. 6. For each human body embalmed:

      1. a. The name of the deceased;
      2. b. The date of death;
      3. c. A statement of whether an autopsy was performed; and
      4. d. The supervising embalmer’s signature and license number;
   7. 7. A statement signed by the intern, apprentice embalmer, or embalmer’s assistant verifying the information on the report is true and correct;
   8. 8. A statement signed by the responsible funeral director verifying the intern; apprentice embalmer, or embalmer’s assistant has been employed by the responsible funeral director; and
   9. 9. A statement signed by the supervising embalmer verifying supervision of the intern, apprentice embalmer, or embalmer’s assistant.
