1. 1. If the license renewal date falls on a Saturday, Sunday, or legal holiday, the licensee may file the application on the next business day following the license renewal date.

2. 2. A renewal application consists of:

   1. a. A form available on the Board’s website that contains:

      1. i. The establishment’s name;
      2. ii. The licensee’s license number; and
      3. iii. If the licensee is an individual or partnership, the signature and tax identification number of the licensee or if the licensee is a corporation or limited liability company, the signature of the authorized signer and the tax identification number of the corporation or limited liability company;
   2. b. If the documentation submitted at the time of initial licensure was not a non-expiring work authorization, documentation specified under A.R.S. § 41-1080 that the establishment licensee’s presence in the United States continues to be authorized under federal law; and
   3. c. The fee required in R4-10-102.

An establishment licensee shall annually submit to the Board an electronic application for renewal on or before the license renewal date.
