1. 1. A copy of a legal document that establishes the name change; or
2. 2. A copy of two forms of identification, one of which includes a picture of the applicant or licensee, reflecting the changed name.

A licensee or an applicant shall notify the Board in writing within 30 days after the applicant’s or licensee’s name is changed. The applicant or licensee shall attach to the written notice:
